Intake New or Existing Patients

Add New Patient

To add a new patient, go to your dashboard or the patients' page, click “+ Add Patient” and fill out the required form. All fields with a red asterisk is required information and must be filled in. 

Intake Existing Patient

To intake an existing patient, select the Alumni or Discharged patient you would like to intake from the Patients page. Click the Intake button and fill out the required form.

Note: Keep the default selection for ERP Program as “Pre-Intake” to enable day 1 billing.


You will notice the Schedule Intake Assessments checkbox is automatically selected. This option must remain checked. All Intake Assessments are automatically scheduled and available in TAP or HUB immediately after adding the patient.



Pre-Intake Checklist

The Pre-Intake Checklist is found on the Patients Profile page. You can complete the items in any order, except for the Clinical Evaluation, which can only be completed after intake assessments are completed, and must be completed by a licensed professional. 

Administer Intake Assessments

Initial intake assessments can be administered on HUB or TAPs. Search for the patient on either interface to complete their assessments.

Note: When Assessments are being taken by a patient, the system is locked down so the patient will not be able to view other patient's info. The system will ask for the user to log back in after assessments are completed to prevent unwanted access.

Assign Staff to Patient

This step during Pre-Intake is not required, but it is recommended. Press Assign Staff to select the patient’s clinical staff member and continue pre-intake.

Set ERP Program and LOC

Select an ERP Program and LOC for the patient. Learn more about ERP Programs here.

Note: Don’t see the LOC you need? Levels of Care need to be added to each Facility, contact your Client Success Manager.

Complete Clinical Evaluation

A licensed clinician needs to complete the patient’s initial Clinical Evaluation. Learn more about clinical evaluations here.


You have now completed Patient Intake and the patient and will start regularly scheduled assessments weekly.
