How to Find a Patient Profile
There are multiple ways to search for your patient's profile.
Go to Patients on the Main Menu, select the patient you would like to view and press View Patient Profile.
You can also access a Patient Profile by searching for the desired patient in the Quick Actions Menu.
Patient Profile Interface
On the top right of the Patient Profile you will find important actions such as intake, discharge, export UR, and transfer.
The first section on the Patient Profile contains the Patient Profile Menu, patient treatment info, basic info, if they are an MAT patient, ERP ID number and level of care.
Press View More to view all patient info including demographics, insurance, and more.

You are able to access different parts of Patient Profile using the Patient Profile Menu.
Patient Overview (Read on below to learn more)
Patient Overview
The Overview page is the main access point of the Patient Profile and contains a full overview of an individual patient's data.
Tasks Due
The Tasks Due section shows you what actions are due for the patient. You may find clinical forms that require a clinician to fill out or assessments for the patients to complete. Click the link associated with each to start the task.
Note: You cannot choose which assessment to start with from the Tasks Due section. If you want to select a specific assessment to administer or start with, go to the Assessments & Clinical Forms page on the Patient Profile Menu.
Add New Task
You can add a new assessment or clinical form to a patient's Due Tasks by pressing + Add New Task.
Completed Assessments & Clinical Forms
The Completed Assessments & Clinical Forms section contains a full log of every assessment and clinical form being completed in real time. Click View All to access the patient's full history of completed and missed forms.
Latest Results Summary
The Latest Results Summary shows the latest results for any given assessment. This is a quick view of the latest and most relevant assessment results for any one assessment. For example you will only ever see the latest Holistic Hexagon score in this section, not multiple scores. Click View All to access all latest assessment results.
Assigned Staff
You can view and manage a patient's assigned staff from the Assigned Staff section. You will also see a full history of all staff members that have previously been assigned to the patient.
Click + Add Staff to assign a new staff member to the patient.
Treatment Info
View and edit which ERP Program a patient is in (learn more about ERP Program Tracks here) and change their assessment schedule from the Treatment Info section.
Level of Care
View and change the patient's current Level of Care and view their LOC history.
Facility History & Housing
View when and by whom a patient was intaken, transferred and discharged from respective facilities.
Diagnosis, Drugs of Choice, and Medications
These three sections are at the bottom of the Patient Overview and can be expanded or closed by pressing the arrow on the top left of each of these sections.
