S.M.A.R.T. Goals were designed to make achieving goals more realistic by adding concrete elements such as resources and actions to the goal setting progress.

A patient can only have one S.M.A.R.T. Goal set at a time so they can focus on that one goal. 

Add a S.M.A.R.T. Goal

Go to the Patient personal page and you will see an + Add S.M.A.R.T. Goal button in the Current S.M.A.R.T. Goal section shown below. You will also see all previous S.M.A.R.T. Goals on the bottom of the page.

Type in a goal and a due date, add resources and actions and press Save Goal.
