Clinical Forms Overview

There are several types of Clinical Forms in the ERPHealth system:

Clinical Evaluations  Learn more about Clinical Evaluations here.

Clinical Impressions: Clinical Impressions are weekly updates on a patient's progress throughout their treatment journey. 

Where to Find Clinical Forms

There are 3 ways to find Clinical Forms on HUB: Through the Quick Actions Menu, the Due Evaluations page, and the Patient Profile.*

You can also complete Clinical Evaluations (not all Clinical Forms) from TAP. Learn more here.

Due Clinical Forms Page

All Due Clinical Forms will appear on the Due Clinical Forms page which you can access from the HUB Main Menu.

Patient Profile

 Clinical Forms that are due for a specific patient can be found on their Patient Profile under Due Tasks. 

Quick Actions Menu

Due Evaluations will also appear on the Quick Actions Menu sidebar on HUB.

Send Clinical Form Links

Clinical forms can also be administered on a clinician's personal device. 

For a clinician to receive Clinical Form links to their phones, they will need to have their phone number verified in Account Settings.
