If you want to change your facilities assessment schedule, contact your Client Success Manager.

There are 2 Levels of Care available in the Continued Care program: Continued Care Monthly and Continued Care Weekly.

Continued Care Monthly LOC

The following assessments are assigned monthly and remain active for the entire month: 

• PHQ9
• GAD-7
• PSS
• Holistic Hexagon
• GSE
• Substance Craving Scale
• Readiness to Change
• PC-PTSD-5

Continued Care Monthly LOC

The following assessments are assigned and remain active Mon-Sun: 

• PHQ9: Bi-Weekly
• GAD-7: Bi-Weekly
• PSS: Bi-Weekly
• Holistic Hexagon: Weekly
• Readiness to Change: Weekly
• Substance Craving Scale: Weekly
