Main ERPHealth Assessments


The following are assessments that are automated by ERPHealth on a regular basis dependent on program and LOC.


Core 4:


Holistic Hexagon

This assessment is a quality of life scaling exercise that is administered weekly to determine where the patient is scoring in 6 categories that have been correlated with improved quality of life and overall well- being. The categories are as follows: Family, Friends, Recreation, Mental Health, Physical Health and Occupation. Quality of life (QoL) is increasingly recognized as central to recovery in patients with substance use disorders (SUD).


General Anxiety Disorder-7 (GAD-7)

The 7-item Generalized Anxiety Disorder Scale (GAD-7) is a self-report scale used to measure GAD symptoms. This assessment is administered every 2 weeks to determine the patient’s levels of GAD. GAD is one of the most common mental disorders and is highly correlated with substance use disorder (SUD). Knowing the patients levels of anxiety enables the providers to create care plans and intervention strategies that will help to reduce anxiety and prevent relapse. The GAD-7 is a valid and efficient tool for screening for GAD and assessing its severity in clinical practice and research.


Perceived Stress Scale (PSS)

This assessment is administered every 2 weeks to determine the patient’s stress levels. Stress is a precursor for depression, anxiety and substance use disorder. Knowing the patient’s levels of stress enables the providers to create care plans and intervention strategies that will help to reduce stress and prevent relapse. PPS items evaluate the degree to which individuals believe their life has been unpredictable, uncontrollable, and overloaded during the previous two weeks.


Patient Health Questionnaire-9 (PHQ9)

Patient Health Questionnaire (PHQ-9) provides clinicians with concise, self-administered tools for assessing depression. As depression is highly correlated with substance use disorder, diagnosing it quickly will enable providers to create treatment and management plans that will help to reduce symptoms. PHQ9s are administered every other week.

Adverse Childhood Experience (ACE)

This assessment is administered once upon intake to determine how many adverse childhood experiences occurred during the patient’s life. Knowing the patients ACE score enables the providers to more quickly identify underlying trauma that may be perpetuating the persons substance use disorder (SUD). The ACE assessment provides validated, objective data, reducing clinical subjectivity and guides both care and care management.


Alcohol Use Disorders Identification Test (AUDIT)

The AUDIT (Alcohol Use Disorders Identification Test) is a simple and effective method of screening for unhealthy alcohol use, defined as risky or hazardous consumption or any alcohol use disorder.


Brief Addiction Monitor (BAM)

The Brief Addiction Monitor (BAM) is a 17-item, multidimensional, progress-monitoring instrument. for patients in treatment for a substance use disorder (SUD). The BAM includes items that assess. risk factors for substance use , protective factors that support sobriety, and drug and alcohol use.


General Self Efficacy (GSE)

The General Self-Efficacy Scale is a 10-item psychometric scale that is designed to assess optimistic self-beliefs to cope with a variety of difficult demands in life. In contrast to other scales that were designed to assess optimism, this one explicitly refers to personal agency, i.e., the belief that one’s actions are responsible for successful outcomes.


PTSD Screen (PC-PTSD-5)

The Primary Care PTSD Screen for DSM-5 (PC-PTSD-5) is a 5-item screen that was designed to identify individuals with probable PTSD in primary care settings. The measure begins with an item which assesses lifetime exposure to traumatic events. If a respondent denies exposure, the PC-PTSD-5 is complete with a score of 0. However, if a respondent indicates that they have had any lifetime exposure to trauma, the respondent is instructed to respond to 5 additional yes/no questions about how that trauma exposure has affected them over the past month.


Readiness to Change

The readiness to changes assessment is used to determine how ready the patient is to change the behaviors related to substance use disorder (SUD). Based on the transtheoretical model of behavior change, it assesses an individual’s readiness to act on a new healthier behavior and informs the provider on how to best guide the change. Readiness to Change assessments are administered every other week.


Substance Craving Scale

The Brief Substance Craving Scale is an 8 items self-report tool that measures intensity and frequency of cravings for primary drugs of choice.

The following are assessments that are not automated by ERPHealth but are available as Off-Scheduled Assessments.


AHC Health-Related Social Needs Screening Tool (AHC HRSN)

The AHC-HRSN is a 10-item screening tool, with 16 supplemental questions, to identify patient needs that can be addressed through community services in 4 domains (economic stability, social & community context, neighborhood & physical environment, and food).


Balance Assessment

This weekly self-reported assessment is a 5 point scale that is based off of the dimensions of ASAM. It was designed to more accurately determine where a patient currently feels they are on the continuum. The scale measures cravings, physical health and wellness, mental and emotional health, relationship to relapse and recovery resources.


Biopsychosocial Assessment (BPS)

There are two parts of the BPS - one part for the patient to complete on their own, and a fuller version for clinicians. BPS assessments can be turned on or off for the facility. Note: BPS can be automated for pre-intake but is off by default.


Borderline Evaluation of Severity over Time (BEST)

The Borderline Evaluation of Severity Over Time (BEST) was developed to rate the thoughts, emotions, and behaviors typical of BPD.


Columbia-Suicide Severity Rating Scale (C-SSRS)

The Columbia-Suicide Severity Rating Scale is a questionnaire used to assess suicide risk.


Drug Abuse Screening Test (DAST-10)

The Drug Abuse Screen Test (DAST-10) was designed to provide a brief, self-report instrument for population screening, clinical case finding and treatment evaluation research. It can be used with adults and older youth. The DAST-10 yields a quantitative index of the degree of consequences related to drug abuse. The instrument takes approximately 5 minutes to administer and may be given in either a self-report or interview format. The DAST may be used in a variety of settings to provide a quick index of drug abuse problems.


Eating Concerns (SCOFF)

The SCOFF questionnaire is a simple, five-question screening measure to assess the possible presence of an eating disorder.


Hamilton Anxiety Rating Scale (HAM-A)

The HAM-A was one of the first rating scales developed to measure the severity of anxiety symptoms, and is still widely used today in both clinical and research settings. The scale consists of 14 items, each defined by a series of symptoms, and measures both psychic anxiety (mental agitation and psychological distress) and somatic anxiety (physical complaints related to anxiety).


Lifestyle Assessment

Essentially, a lifestyle assessment helps a person be informed and proactive with their health. It teaches how lifestyle choices impact health outcomes. It also helps the doctor be more informed about their patients' health and gives them the opportunity to create an action plan.


PTSD Scale (PCL-5)

The PTSD Checklist for DSM-5 (PCL-5) is a 20-item self-report measure that assesses the presence and severity of PTSD symptoms. Items on the PCL-5 correspond with DSM-5 criteria for PTSD. The PCL-5 can be used to quantify and monitor symptoms over time, to screen individuals for PTSD, and to assist in making a provisional or temporary diagnosis of PTSD.


Psychiatric Medications Questionnaire

Used to measure a patient's adherence to or abuse of prescribed medications.


Brief Assessment of Recovery Capital (BARC-10)

The BARC-10 is a short, 10-item measure that examines recovery capital globally. Items were selected from the ARC for the BARC-10 using item response theory. The BARC-10 measures a unidimensional (i.e., global) construct of recovery capital across all of the original 10 domains of the ARC.


Sexual Compulsivity Scale

The Sexual Compulsivity Scale is a psychometric measure of a high libido, hypersexuality and sexual addiction. It was developed by Seth Kalichman. It consists of statements that must be rated on how much the taker agrees with them.


Stroop Test

The Stroop Color and Word Test (SCWT) is a neuropsychological test extensively used to assess the ability to inhibit cognitive interference that occurs when the processing of a specific stimulus feature impedes the simultaneous processing of a second stimulus attribute, well-known as the Stroop Effect.


Basis-24

The 24-item Behavior and Symptom Identification Scale (BASIS-24) is a widely used behavioral health self-assessment tool to measure symptoms in 6 domains: depression and functioning, relationships, self-harm, emotional lability, psychosis, and substance use. This assessment can only be activated by request with proof of license.
